Term and conditions

1. Nature of Agreement

By ticking the "I Accept" box and submitting this service request, you (the "Customer/Patient") enter into a legally binding agreement with MAHEE Home Health Care Services. This agreement governs the provision of medical and non-medical home care services.

2. Scope of Services Care Plan: Services will be provided based on the initial assessment and the agreed-upon Care Plan. Clinical Supervision: All nursing tasks will be performed under the supervision of a registered professional or as per the treating doctor's prescription. Modifications: Any changes to the frequency or type of care must be requested at least 48 hours in advance and may result in a rate adjustment. 3. Billing and Payment Service Rates: Charges are based on the specific category of service (Nursing, Physiotherapy, or Caregiving) as displayed at the time of booking. Payment Schedule: Payments are due [Select one: In Advance / Weekly / Monthly]. Late Payments: MAHEE reserves the right to suspend services if invoices remain unpaid for more than [3] days past the due date. 4. Cancellation & Refund Policy Cancellations: A minimum of 24 hours’ notice is required to cancel or reschedule a shift. Cancellations made with less than 24 hours' notice will incur a fee equivalent to [50/100]% of the shift cost. Refunds: Pre-paid amounts for unrendered services (provided the cancellation policy was met) will be refunded within [7-10] working days. 5. Customer Responsibilities Safe Environment: The Customer must ensure a safe, hygienic, and harassment-free environment for MAHEE staff. Medical Supplies: Unless otherwise agreed, the Customer is responsible for providing necessary medical consumables (e.g., gloves, gauze, masks). Accurate History: The Customer must disclose all known allergies, infectious diseases, and medical history to ensure staff safety and effective care. 6. Non-Solicitation (Private Hiring) The Customer agrees not to privately hire or contract any MAHEE staff member introduced to them for a period of 12 months following the termination of this agreement. Violation of this clause will result in a placement fee of [Insert Amount]. 7. Liability & Privacy Liability: MAHEE is not liable for complications arising from the patient’s non-compliance with medical advice or pre-existing conditions. Data Privacy: MAHEE complies with data protection laws. Your personal and medical information will only be used for care coordination and billing purposes. 8. Termination Either party may terminate this agreement by providing [3] days' written notice. MAHEE may terminate immediately in cases of non-payment or an unsafe working environment.

I hereby certify that I have read, understood, and agree to the Terms and Conditions of MAHEE Home Health Care Services. I authorize MAHEE to provide care as per the agreed plan.

For non-medical caregiving and companionship, no referral is needed. However, for clinical services like injections, wound dressing, or catheter care, we require a valid prescription from your treating physician.


Yes. We offer flexible scheduling ranging from short 4-hour visits to 12-hour shifts and 24/7 live-in care.


We have a standby team of "backup" caregivers. If your regular staff member is unavailable, we will notify you immediately and send a qualified replacement to ensure your care is not interrupted.


Absolutely. For long-term care assignments, we can arrange a brief introductory call or meeting to ensure the caregiver is a good personality fit for your family.

Rates depend on the level of care required (e.g., a General Duty Assistant costs less than a Critical Care Nurse). Please fill out our "Service Request Form" for a personalized quote based on your specific needs.
